Script Details

When the lesion worsens, it can be seen to extend up the walls of the pit or fissure because subsurface demineralisation is worsening. This is an ICDAS [I-SEE-DAS] code 2 lesion.

If demineralisation continues the structure of the enamel becomes even more porous and fragile, eventually the surface of the lesion breaks down causing small cavities in the enamel surface but at this point the dentine is not yet exposed. This is now an ICDAS code 3 lesion. Clinically, the surface is no longer intact.

As subsurface demineralisation continues, the enamel becomes undermined by the lesion extending further into dentine. At this point, the dentine caries can be seen through the enamel surface as a dark grey or brown shadow. This is sometimes known as hidden dentinal caries because the enamel surface may still be relatively intact. This is an ICDAS code 4 lesion.

Eventually, the enamel surface gives way exposing the carious dentine beneath and a cavity is apparent. ICDAS codes 5 and 6.
