During this video we will be looking at the pre op process, which begins in the physician’s office and ends on the day of surgery. We will review the tasks to be performed by the patient, office staff, and hospital staff to successfully prepare the patient for surgery.”
“The first step of the pre op process is the patient’s appointment with the surgeon. The office staff will contact the operating room scheduler to set a date for surgery, as well as receive a date for the patient’s pre-anesthesia interview.”
“Once appointment dates are confirmed, the office staff will educate the patient on the pre op process. This includes patient registration procedures, appointment dates and times (including any necessary appointments with the patients primary care provider or specialist), and necessary items the patient will need to provide during the pre-anesthesia interview. It is important the patient be made aware of items to take to the pre anesthesia interview, including a current list of medications and any medical ID cards, such as pacemaker or implant cards. Additionally, the patient should be informed that this interview will occur in Same Day Care and will take approximately one hour. Diagnostic tests may also be performed during this time. The patient should be asked not to use the bathroom immediately prior to their pre anesthesia interview, as a urine sample may be needed. Once the patient verbalizes understanding of these instructions, they are ready to move on to the next step of the process, registration.”
“During registration the patient will provide demographic and contact information to the registration staff, as well as insurance and billing information. The patient may receive a call from registration prior to their physical registration visit to obtain demographic information. In order to register the patient, an order for surgery must be present, as well as insurance pre certification (or documentation that the process has been initiated). If an order is not present the patient cannot be registered. This process usually takes approximately 10 to 30 minutes. Typically, a patient will register on the same day of their pre anesthesia interview. If a patient chooses to register immediately prior to their pre anesthesia interview, it is vital they arrive 20-30 minutes before their appointment. This allows time for the registration process to occur without cutting the pre anesthesia interview short. When the patient has finished registering, they are now ready to move to the next step, the pre anesthesia interview.
“Upon arrival to same day care, the patient will ring the bell at the window for assistance and let the same day care staff know they have arrived for their pre anesthesia interview. They will give the Health Unit Coordinator (HUC) the documents they received in registration, and be asked to have a seat until the nurse is ready to see them. The HUC will then give the documents to the nurse as she prepares to see the patient.”
“Once the patient has reached the pre anesthesia interview office, the nurse will begin the interview process. This includes a thorough review of the patient’s medications and health history, as well as the facilitation of any diagnostic tests ordered (including lab work, EKG, urine specimen collection, and radiology services when applicable). It is essential that the patient have a current and complete list of medications for the nurse to review during the interview. Following this review, the patient will sign a consent for surgery and anesthesia services, and speak with an anesthesia provider. The anesthesia provider will further review the patient’s medications and health history. They may also order additional diagnostic tests and/or medical clearance as needed. After being seen by anesthesia, the patient will be given verbal and written instructions regarding day of surgery expectations. The patient has now completed their pre anesthesia interview. Please remember, it is important the patient is aware that this process takes approximately one hour to complete.”
“Following the pre anesthesia interview, the Same Day Care staff will review the patient’s chart to ensure that all necessary steps have been completed. The patient will be contacted the business day prior to surgery to verify their arrival time for surgery. For the patient to have surgery, there must be a History and Physical completed within 30 days of the surgical date, an order for surgery from the physician, and any medical clearance or diagnostics that have been requested present on the patients chart. Once all required documents are present, the chart is approved and ready for the day of surgery.”
“If there are any components missing, or the patient does not meet criteria for surgery per the hospital’s policy then the physician’s office will be notified. Examples of this would be abnormal lab results or other abnormal diagnostics, medications not held within time frame, medical clearance not received as ordered, or a change in the patient’s health status between their pre op interview appointment and day of surgery. If these discrepancies are not resolved either by the Same Day Care Staff or Physician’s office the surgery will be cancelled.”
“The day of surgery marks the completion of the pre-op process. All steps have been taken by both the physician’s office and hospital staff to ensure that the patient is ready for surgery. There is a physician’s order and history and physical on the patients chart with appropriate dates, the pre anesthesia interview has been completed, and all necessary medical clearances are present on the chart and signed. The patient has completed all lab and diagnostic testing ordered prior to surgery and has clear instruction regarding day of surgery expectations. Through this multi-step process, any discrepancies that may cause the patient to have a delay in or cancellation of surgery are addressed, ensuring that the patient has a safe and successful surgery experience at Murphy Medical Center.”
