The terminology is not very medical or scientific however we are looking for someone who has a smooth delivery and can speak as if they were a knowledgeable, caring and compassionate doctor. Yes. I think it is very important. I think the most important thing, of course, as a physician is to deliver the best care that we can, to continuously improve our education, learn from our colleagues and importantly learn from our patients, but having done that, the question then becomes how do we know how we are doing? Are we applying the most up-to-date evidence in our care? Are we addressing the patients’ needs? Some of that comes from our encounters with our patients but some of it very well may not and I think that is where quality improvement comes in. Quality improvement tries to support our physicians to become better. It tries to support the institutions in which they work to operate better for them and their patients and I think the Hallmark principle for quality improvement is not to think of it as something that is targeting the physician but it is targeting the environment in which they are trying to deliver the best care that they can. So, typically quality improvement is focusing on improving the operation of the clinic, trying to improve the systems that support the doctor, trying to make things available to the doctor while they are seeing the patient as opposed to identifying the “bad doctor.” That is very rarely what the outcome of quality improvement is and I think that is one of the major misconceptions nationally and internationally about why do we do what we do.
