We’ve developed technology-enabled programs that identify high risk patients and empower them to take charge of their condition through timely education, peer support and continuous access to world-class care.
Our Modality Choice Program is based on peer-reviewed, proven approaches.
It starts with a self-assessment to identify values, concerns and motivations.
Learning our patients’ stories helps us and their providers give more effective, personalized care.
Then, patients learn about treatment options through short and personalized videos explained by other patients and experts.
We give patients a dedicated Cricket Nurse who is proactive in their care and accessible anytime.
We also connect patients with mentors who can share their treatment experience.
And we bring patients together to reinforce learning and provide encouragement and support.
By the end of the program, patients are equipped to make a well-informed, timely modality choice.
They get a short, structured summary that they can use to discuss their choice with their physicians or loved ones.
After completing the Modality Choice Program, we continue to support patients through our Continuous Care Program.
They get the education, care and connection they need between visits, delivered by our online platform and care team.
We’re there for patients throughout their CKD journey-- proactive, accessible and ready to help.
