Delayed access to hospice and palliative care services occurs for a variety of reasons related to patients and families
First, The Services are not well understood
Lack of understanding of what comprehensive palliative care programs offer leads to confusion over when it is appropriate to consult or transfer care to these services. More timely referrals are necessary in order for patients and families to reap the full benefits of hospice and palliative care services.
Surveys indicate that patients prefer to die at home, yet only 29% of terminally ill patients enroll in hospice programs . The numbers are higher approximately 50% for cancer patients. Even less frequently utilized in long-term care: 13% of hospice enrollees were in nursing homes; 70% of nursing homes had no hospice patients at all.
Hospice and palliative care services offer aggressive non-curative treatments and interventions. Patients and families do not feel abandoned. They receive support and care based on their unique goals and needs.
Next, Rules and regulations may prevent enrollment in hospice services.
Institutional regulations that impede good EOL care include restrictive visiting hours and inadequate policies for pain and symptom management.
Regulation of controlled substances has led to fear of prosecution for prescribing and administering medications to relieve pain and other symptoms.
Issues regarding access to care, insurance coverage, and the potential need to hire a caregiver from outside the family contribute to financial barriers to care
finally Denial of death is a barrier.
Reluctance to take away "hope" may be one of the biggest barriers in accessing needed services
Poor communication about patient and family preferences can impede timely referrals to comprehensive end-of-life services.
