The motor control system consists of the cortical areas, the subcortical areas including the basal ganglia and the cerebellum, the brain stem, and the spinal cord. Lesions within the brain resulting from brain injury, cerebral palsy, and stroke result in cortical spasticity, while spinal spasticity results from lesions in the spinal cord. Cortical-mediated spasticity usually occurs due to enhanced excitability of the monosynaptic pathways, but spinal cord-mediated spasticity usually involves polysynaptic responses and reduced presynaptic inhibition of Ia afferent fibers. Although there appears to be some difference between cortical and spinal-mediated spasticity, they do not have a significant influence on the clinical features and impairment of function.
