Donning and Doffing of
Personal Protective Equipment
for Hospital Personnel.
The following is a review of the proper procedures for DONNING, "putting on" and DOFFING, "taking off" the personal protective equipment (PPE) that hospital personal are required to use during a HAZMAT / DECON operation.
DONNING PPE
The first step would be to gather your equipment together.
This would include the following for each member to be suited:
1 Chemical protective suit, yellow Tychem BR
1 Pair HAZMAT boots
1 Pair butyl gloves
1 Pair nitrile under glove
1 PAPR with battery
1 PAPR hood.
1 Roll of CHEM-TAPE
Once assembled, you should inspect chemical suit for damage or defects.
Similarly, you should inspect the PAPR hood and hose for any defects or damage.
Connect the battery to the PAPR and turn the unit on.
Check that the unit's motor is operating.
Turn unit off, disconnect the battery from PAPR, and set aside.
In normal conditions it will take two persons to suit - up one individual.
Before suiting up personnel, the Decon Team Safety Officer should complete a pre-entry physical. This physical should document the following:
Pulse, Blood Pressure and Respiratory Rate.
Decon team members should hydrate with a minimum of 1 liter of water.
Once the physical condition of personnel is determined, the team members being suited should be seated. The proper size suit and boots should be gathered.
While seated, the team member shall remove his/hers shoes. Any personal effects, for example a wallet, watch, rings or any other jewelry should be removed and secured.
While remaining seated the team member shall don the chemical suit over each leg. Caution should be taken not to over stress the lower end of the suits zipper.
Once the suits legs have been donned a support team member shall assist with the donning of boots.
The outer lower leg cowling should be pulled up. Gather the suits excess bootie material toward the heel of the foot.
Carefully pull boot over foot, secure the bootstraps and pull the cowling down over top of boot. Repeat the same for the over foot. NO TAPE IS REQUIRED OVER THE BOOT / SUIT INTERFACE.
Have the team member stand and insure the boots and suit are comfortable.
Team member should place arms into protective suit.
Proceed with hand protection:
Have team member don nitrile under gloves.
The suits wristbands should be pulled down over gloves.
Next, support team members shall assist with donning of butyl over gloves.
Butyl over gloves shall be pulled up over the wristbands of the suit
These gloves shall be secured in-place with chem-tape.
This is the wrong way to apply the tape.
A tab should be placed at one end of the tape.
The chem-tape should cover 50% of the glove and 50% of the suit in the area where the glove overlaps the suit.
A support team member shall secure the hood piece that is integral to the chemical suit. This hood shall be rolled up and tucked inside the collar of the suit.
The following procedure will require the cooperation of two support team members:
The protective suit shall be zippered 3/4 of the way up.
The chemical suit has two overlapping flaps. They have tape bands on the inner edge. This tape band should be removed and the flaps secured, one over the zipper, one over the first storm flap.
A final long strip of chem-tape shall be place over the second storm flap. 50% on the flap and 50% on the suit.
The first support team member shall assist with the donning of the PAPR motor unit.
The team member shall connect the air tube to the PAPR hood by snapping the white male connector into the white female connection on the PAPR hood.
The battery is connected and the PAPR turned on.
Drape the PAPR hood and hose over the team member's shoulder, while holding the PAPR motor to the small of the suited team members back with one hand, and the battery with the other hand.
Secure the PAPR belt around the member and secure the battery to the belt.
Once the belt is adjusted and the PAPR motor is tight to the wearers back,
Secure the excess belt by folding it up and tucking it under the belt at the waist.
The second support team member shall assist the team member in donning the PAPR hood.
The PAPR hood has both an inner and outer cowling. The inner cowling is intended to go under the chemical suit.
The support team member shall stuff the inner cowling under the shoulder, around the collar and down the back of the suited team member.
Once the inner cowling is secure under the chemical suit the zipper may be advanced to its top, under the chin of the suited member.
The outer cowling should be pulled down into place.
Assist the suited member with the adjustment of the hood's headband.
Complete a final inspection of all tape to suit interfaces. Smooth out any kinks in the tape. Make final check on PAPR and battery.
The donning of PPE is complete.
DOFFING PPE
The doffing of PPE will require two support team members. The doffing of PPE is basically the reverse process of donning.
All suited team members shall have been thoroughly DECONED before doffing procedures may begin.
The support team member should be wearing nitrile gloves and goggles, this is the only protection needed. They should have a chair or other seating available for team members being doffed.
They member to be doffed should not be allowed to sit until the chemical suit is pulled down around their legs.
Doffing should start with gloves. The outer butyl gloves are un-taped and removed. The inner gloves should remain in-place for now.
The suited team member should be directed to keep his/her hands up in front of themselves and not to touch the support team members or their suit.
With the assistance of a second support team member holding the PAPR motor and battery, the PAPR belt may be released. The second team member shall support the PAPR motor and battery throughout the remainder of the doffing procedure. (Respiratory protection is the last thing to be doffed)
The tape securing the front of the suit (the storm flaps) is the next thing to be removed. The storm flaps are then opened and the suit unzipped completely.
The chemical suit is then rolled off the shoulders, one side at a time. The chemical suit should be rolled inside over outside. In other words, the clean inside of the suits should be rolled over the dirty outside. Just remember, clean over dirty.
Once the shoulders are free, the support member shall assist the suited member in removing their arms, one at a time.
The support team member should open the wristbands at the bottom of the sleeve to allow the hands of the member to pass freely through the sleeve.
Once the member's hands are free they should once again be place in a position out in front of the member. The same steps should be repeated to remove the other arm.
Once the member's arms are free the entire top of the suit shall be rolled down (remember clean over dirty) to the top of the members knees.
It is at this time that the member may be seat to facilitate the removal of their boots.
The support team member shall now remover the boots from the member's feet.
Once the boots are off the suit may be removed from the member's legs.
The member may now remove the nitrile under gloves and discard them.
Finally, the member reaches under the inside cowling of the PAPR hood and pushes the hood over and off their head.
The support team member holding the PAPR motor and battery will take the hood from the member.
The doffing procedures are now complete.
The Safety Officer shall now conduct a post entry exam. Vital signs should be documented as before and the member should re-hydrate him or herself.
